For your pet’s protection, Bordetella and Rabies must be current *

My pet's Bordetella and Rabies vaccinations are current.
I am unable to provide proof of vaccination and give permission to update my pet’s vaccinations in accordance with Odom Veterinary’s policy. I understand this may include a required examination fee ($45).

If any fleas/ticks are observed on your pet while boarding, he/she will receive a Capstar at the owner’s expense. *

I authorize my pet to receive a Capstar treatment at my expense if fleas/ticks are observed during boarding.

Agreement and understanding of potential abandonment if pet is not picked up within 5 days.

I understand that the continuous presence of qualified personnel may not be provided. If I neglect to pick up my pet within 5 days of the discharge date and do not notify you within that time period, you may assume that my pet is abandoned, and I hereby authorize you to dispose of my pet as you deem best and/or necessary.
